Calculation Scripts disappearing

Hey everyone,
 
I have created a self assessment tool that displays one of three different blocks of text on the results page, depending on the answers that the user has chosen.
 
As far as I can tell this works fine on my PC but when a user downloads the tool the calculation scripts disappear. Are there any possible reasons as to why this should be happening?
 
Also, a friend who has a mac and is running Adobe X can't even fill the form in!
