Cannot Fill in PDF form

I am trying to fill in a form online, access the form @ [link=http://www.orhp.com/service_contractors/contractorservice.pdf]http://www.orhp.com/service_contractors/contractorservice.pdf[/link]
I have saved it to my computer also, and when I try to type in the fields, the cursor is there, but when I start typing, nothing happens, and when I hit the enter or space bar it just hops all over the page?? I have checked the securtiy on this document, and it shows to allow this action.
I am needing to get this form in asap, so any help would be appreciated.

Hi beckylouisiana,

The only page on this form with fillable fields is the W-9 page. None of the other pages have form fields- that is why you can't just place your cursor in those areas and type into them.
You don't say what program you are using to view the form- the free Adobe Reader or Acrobat (not free). If you have Acrobat Professional 7 or later you could use the Typewriter tool for a quicker job of filling in the information, or you could spend the time to add fillable fields yourself using the forms tools (this would take you a while longer).

If you are using the free Adobe Reader, the only option is to print the form and fill it out by hand, then scan it or send it back via snail-mail. If the site you downloaded this form from says it is fillable, then you should contact them and let the author know it is not- maybe they published an incorrect version.

I'm having the same problem as "beckylouisiana" in that the online form will not allow me to enter data but in my case, it will allow me to click on the check mark boxes. I'm assuming that you will tell me to let the author know and I will...but I just want an extra jump on the other applicants. I've already converted the form with PDFFill but and I know that I can fax it...but again, I want to appear more resourceful than that. (Asking for help is being resoureful!)

Unless you have Acrobat 9 standard, Acrobat Professional, or Nitro PDF you wil not have a form authoring product. And that is why we reccommend that people contact the author.
