My form has a performance rating requirement and the person is to choose their rating of 1 through to 5 for each row. I used combo boxes to create the ratings from 1 to 5. It lets me choose which rating I want to show for that row but, when I choose the rating for the next row, they all change to show the same rating.

What am I missing?

You need to give each field a different name.
