Filling out PDF forms

Ok, I thought I was pretty smart. Downloaded a PDF doc (employment app) and I have tried everything to fill is out. The "fill out forms" is "allowed" in the security settings, but I can't get boxes to come up. I'm using Adobe Reader 9.0. Did the trial version of the Adobe Pro and was able to fill in fields, but alass since it's the trial version it won't print the information I typed (glad I had the sense to test print the first part of it. New here and any help would be appreciated. Tried to copy past in MS Word 2007 (breaks up the quality of the form and Publisher. Thanks in advance for you help.

Are there form field on the form?

Just because someone scans a form to a PDF does not mean that the necessary form field's have been added to the scanned image. And the security setting that allows for filling in forms has no real affect if there are no form fields.

Can you provide a link to the form?

Thank you for your reply. The document is "scanned" and I think that is why it does not have fields. Here's the link https://www.morehead.org/Portal/Main.aspx?tid=244&mtid=1 click on "down load document" and that's the one.
I'd think that they would be more technical savvy for a hospital. My neighbor has a program that inserts fields, thankfully, too bad, no body has a typewriter anymore.
