I have created a form using Adobe Acrobate Professional 8. I have created a "submit" button for signing the form. I do not have any security set. The end user has a windows digital signature for signing the document. When the end user (using Reader 8) attempts to sign the document the digital signature comes up and when she clicks "sign" she gets the error "sign operation failed." I've tried adding security and removing security...no change. Thanks for the help..

Have you Enabled the form so Digital Signatures can be used in Adobe Reader? Digital Signature capability in Reader is one of the Usage Rights that must be applied to the PDF.
See this article by Ted Padova on how to apply Usage Rights to the PDF-

Also, if you do apply security to the document it must be done before Enabling Usage Rights- Enabling is the last step in the process or it could be invalidated.
Hope that Helps,
