I need a form that can be used as a course evaluation that will be completed at the end of a course by our students. This form would ask questions on their opinion of the class and they would fill in the "bubble" in ink (choices 1 through 4). Would then like to scan the form and collect the data. Can I do this will Adobe Acrobat 8?

You can certainly create the form, but there is no mechanism in Acrobat to collect pencil-filled circles from paper.
