I am using Adobe Reader and have filled out an insurance application on the form, but I can't get the form to print nor can I clear the form to type another application on to it. Can you help?

I'll go out on a limb and say the form designer screwed up. In general, forms should have a reset button. You can't add one with Reader--you need Acrobat Professional. Or perhaps the cache is still holding your form data.
Reader does have the "Save a Copy" feature. Check the security settings to see if printing is permitted.
