I am trying to develop a series of forms for a medical practice. These forms have several sections including personal information, medical conditions, health insurance info, medications etc.
I would like to present the form questions in a number of small groups that are easy to read and fill out (submit) For example all personal/contact info might be on one page (on the monitor)...rather large..easy to read...easy to fill out.
Once the patient has filled out all the samall groups of questions they will need to printed in a more compact form so that the office has a copy. Obviously we dont want to have one page for each "section". We want all the sections to be scaled down into a more concise document.
Is this possible within Acrobat?
I know using a simple html form I could save them to a database but then I'm not sure I could that info to be printable on a more compact printed form.

Which of these methods is most secure?
For the custom made script I would need someone to write the script correct? IS there a term for this type of scripting?
I would probably try to partner with a scripting programmer to get this done.

None of these methods is very secure. But it depends very much on your configuration.
For a more secure option I would recommend a web-form, which can be securely submitted to a server.
