We need a little more information. There's no such thing as an insert text box tool. If using the Comment tools and using the Text Box tool, you need to be cerain the file is not password protected with permissions restricting commenting. If using Adobe Reader, the file has to be enabled with Reader usage rights.
If creating a form field using the Text Field tool, you need to be in edit mode. Select Forms > Edit Form in Acrobat.ted
