Need to create an editable form for client

I am not very experienced in Adobe Acrobat, but have a client that needs a form to be created for them with fill in fields and check boxes. The plan is to have me create it and they keep the original master form on their system, update as needed and publish as often as necessary.

PROBLEM: I can't design it. I did the original in Quark, made a pdf, and made the fields live. It works, but is NOT editable for them.

SOLUTION: Are there any AA savy designers out there that can help me with this?

In order for your clients to edit the form, they need Acrobat 8 Professional. If they would like the ability to save form field information with Reader, then you'll need to Reader-enable the form with Acrobat 8 Professional.
