I remember that there was a contest for forms, Adobe was collecting Fillable Forms and compiling them then offering a prize for the best one. Where can I post and is it possible to see the entries? I think that it would be great to see examples of forms that other users have created so that we can get ideas or have something to start from to modify to our own style.
I have created Medical S.O.A.P style forms for daily treatment documentation. This could be very usefull to anyone in the medical field, the software that automatically creates forms like these costs thousands of $$$s per doctor, I am willing to give mine up for free if others would comment, reccomend changes and offer other similar examples.
My next task is Patient Registration Forms, there must be dozens of these out there, why should I work from scratch on this?
Any Help? Any medical IT guys out there looking for a similar palce to share ideas?

Keep in mind medical forms need to comply with HIPAA.

This is why I am trying to find out more information on the "Secure Forms on Return" option, but I haven't found anyone yet who seems to have gotten this to work, or can provide more information on what the encryption on the return data is.
