Recipients addding Images to PDF Form

To anyone that can help...

I'm creating a interactive barcode order form for our satellite studios to order barcodes from us.
It will have all the buttons and drop downs and all the validations that I would need to create a barcode without really seeing a sample of how the code looks like.

I do run into those hard to figure out codes from time to time. If I were to make a space into my form for the recipient to attach an image (.eps, .ai, .jpg .tif). Would that be possible?

Once the image is in place they then can submit like normal.

Is the only way to do it to add an additional page to the form and send that with the original form?

You cannot create such a form with Acrobat, unfortunately (and entirely unreasonably). You can create such a form with LiveCycle Designer, which comes with Acrobat for Windows.

You could set up the image field on any page of the form you like. Supported image formats are jpg, gif, png, and tif, so eps and ai are out.

For it to work for Reader users, the document has to be extended for use with Reader (aka Reader-enabled), which has certain licensing restrictions. If you have fewer than 500 users you'll be OK though.
