Turn PDF form into a web form

Hello - I'm trying to turn a PDF form into a webpage that preserves the formatting etc. of the form itself. We have a long survey in PDF form that we spent a few hours meticulously making into an interactive PDF form. For research purposes, we need the form to go online as part of a webpage rather than any type of PDF. I tried the PDF to HTML converter on the Adobe website, but it botched all of the formatting/colors in the original form. Does anyone have an idea of how to do such a conversion? I've been looking into this issue for the last few days and can't find anyone that knows if this is possible.

Thank you, but I'm looking to go from PDF > HTML, not the opposite. I've tried a few different types of conversions, but none of them retain the formatting of the original document (which is critical here).
