Application Error when inporting .pdf form

I keep getting the following error when trying to import a pdf form
"The instruction at "0x019617c9" referenced memory at "0x0694a008". Th memory could not be "written". Click OK to terminate the program.
Is this my computer glitch?

You should check the event log of your OS for more information about.
