need a simple how-to or tutorial for making a data connection to excel

I can't seem to find a really basic or simple tutorial on how to make a data connection to a field in a pdf form in LC Designer ES . I basically want to use an excel document as a lookup table, just like the function vlookup in excel.

But I've looked high and low for an example or description beyond the above. You've posted twice and the same question has been posted on at least four other boards with 0 responses. The PDF/LiveCycle Bible devotes little to Excel (tho Access has examples).
