Phone Message

Hi All,

I'm trying to figure out if this would be a practical project for Adobe Acrobat 9 Pro.

I want to create a Phone Message pdf which would be given to the users in my office that use Adobe Reader. When they take a phone message, the form would contain
a pull down field that gives them the option of selecting the appropriate user to email the message to, as well as other items pertaining to the call ie. creation time & date, importance flag, action to be taken and a automatic email button.

If this is practical how would one go about doing this?
