Problems with Form Created in Live Cycle

I hope someone can help me with a couple of problems I've been having with an interactive PDF form created in Livecycle.

1. I have been able to enter test data into the newly created form with no problem. When I hit the "submit" button it tells me that I must complete the highlighted fields before the form can be sent. There are red highlight boxes around the fields where the data has been entered, and the data is on the screen. Why does it not recognize the data entered and how can I fix it?

2. Second, there is one text field (numeric) that generates a txt error when the data is entered. It's a text box used entering a phone extension number. I have deleted the formatting description from the field settings in hopes that would get rid of the problem, but with no luck. How can I fix this?

I had an issue similar to #1 just recently. It turned out that at some point I had inadvertently made a copy of the field, and it was sitting right on top of (or underneath - hard to tell in two dimensions) the other field. So, though one field had data in it, its doppleganger did not, and thus caused the error, even though it appeared that everything was right.
