Acrobat 9 Replace pages problems

When I try to replace a page in Acrobat 9, sometimes a one or two inch margin on the right side is cut off, showing only the first few letters of the lines to the right. There are no links or anything on the pages and it happens on all pages where there is a chart or graph or anything other than regular text. It does not matter where I try to replace the pages it happens on all documents and pages. I can not seem to find a solution on any Adobe help site either. We used to have to reassign over a hundred bookmarks each week in order to complete this weekly report and when we tried the "replace pages" feature we hoped our problems would end.
 
Is this a problem with the original authoring program or a problem with Acrobat itself?

What is the original authoring program? There can often be several page sizes written into a PDFs document info, bounding box, trim size, crop size & bleed. It sounds like on pages with charts & graphs it is grabbing the live image area, instead of page size, or having issues locating the proper page dimensions. The method you use to create the PDF can also impact Acrobat's ability to read the proper page size. For example, File > Print to Adobe PDF Printer or PDF Maker in Microsoft Office.
