I use Acrobat Pro on a almost daily basis. Here are some real frustrations.
1. Options for OCR
If I "Recognize text in this file", I'm presented with 3 output options:
a. Searchable image
b. Searchable image (Exact)
c. Clearscan
I almost always want the second option; this preserves the original scan without making the file overly bloated.
HOWEVER, if I use "Optimize Scanned PDF", and edit the OCR options, I'm presented with only 2 output options:
a. Searchable image
b. Clearscan
Why can't I have an "Exact Image" option here?
If this has something to do with scanning from within Acrobat, then let's get this repaired. I ALWAYS scan outside Acrobat; I'm often scanning old papers of poor quality and the images MUST be cleaned up in an image editor before they're converted to PDF.
2. Deskew doesn't work
If I use the "Optimize Scanned PDF" and turn deskew on, Acrobat X will NOT adequately deskew scanned images. I have to run Acrobat in Windows to get decent deskewing. I can't use version 9 of Acrobat on the Mac because the optimize function has been broken forever -- if the scanned image contains a halftone, Acrobat 9 on the Mac will utterly clobber the output file. This bug was repeatedly reported to Adobe but never fixed.
3. Why can't Acrobat remember options set by the user?
I use the "Combine Files into a Single PDF" function VERY frequently, to assemble a PDF from a set of scanned pages. If I open Options and deselect "Always add bookmarks to Adobe PDF" (I NEVER want each page to have a bookmark), Acrobat will NOT remember that option. I have to deselect this option EVERY TIME I open the program.
There should be a way to stop annoying users and make options "sticky" if the user prefers that.

It's a shame!
