Combining PDF documents and providing access to individual chapters

I have five PDF documents (created in MS Word) each with five or six chapters. I want to combine them into a single master document (27 chapters) allowing readers to access/download individual chapters by clicking on a chapter name in a master table of contents.
Is this possible and, if so, how can it be done?

After you combine the file you can create bookmarks that lead to the beginning of each chapter.
However, it's not possible to download only a part of a file.
