HELP restore missing Title Bar in pdf

I cannot find the solution to restore my title bar. I dont know what I did - so could someone please tell me (a) what I did to make it disappear and (b) How do I get it back?

Please save my sanity!

File -> Properties, under Iniitial View tab, Window Options- Show, select Document Title.Hope this helps,
