hyperlink issues: Word 2007 to Acrobat 8.1.2

Observations:

Table of Contents generated by Word -- items are off by one, for example Chapter 1 links to Chapter 2; Chapter 2 links to Chapter 3. An invisible link appears in the white space above the first entry; for example above Chapter 1. If one clicks on that white space, it takes you to Chapter One. The conversion appears to be shifting all links up one line.

Other Links. Hyperlinks created in Word that are spelled out, e.g. http://www.abc.com appear to work fine.

Some links that are created in word by by highlighting text and using the Insert Hyperlink command do not work. In particular the first one in the text does not work.

A few links open a Acrobat page instead of opening the browser.

Selected setting are: Add bookmarks to PDF
Add links to PDF

Any ideas on how to resolve these issues?
