Keeping track of emails sent to users

I have designed a form that needs to go to several departments. on the form I have several submit email buttons. When these button are click the form will go to that department.
1. The form get a ID, which is filled in along with a description in dept. A. The submit email button is clicked and a email is sent to dept. B with the form attached.
2. When Dept. B gets the email that person fills in their portion of the form and clicks the submit email button to send it to dept. C with the form attached.
3. When dept. C gets the email that person fills in their portion of the form and clicks the submit email button to send it to dept. D with the form attached.
4. When dept. D gets the email that person fills in their portion of the form and clicks the submit email button to send it to dept. E with the form attached.
5. When dept. E gets the email that person fills in their portion of the form and click the submit email button to send it back to dept. A with the form attached.
What I am looking for is a way to track this form, so I know where it is all the time. Is there away to do this?
