2D barcodes

For simple (2 numeric fields)indexing into an imaging system I'd like to use a 2D barcode. However, the barcode can be preset/prepopulated prior to sending out to our client, who will then ONLY (will not modify it further) view and print the form with Reader 8, sign it and return it to us. Is it necessary to obtain additional usage license from Adobe to accomplish this?
 
Or would it just be easier to have two 1D barcodes on the form?
 
Is this something I can accomplish via FDF?

If the user is printing the form from Reader then it shouldn't need form rights at all. Have you tested this out?
