Hand coding the custom dialogs is a bear but doable- look up the dialog object in the Acrobat JavaScript reference. Or try out my company's plugin AcroDialogs- http://www.windjack.com/product/acrodialogs/ It makes the task of creating full featured custom dialogs much easier.
