dynamic field population

Hello experts!
This is for LiveCycle Designer 8.
 
I have a dropdown list that contains the names of certain clinnics (about 25 items). Next to this dropdown list are textfields that suppose to contain the location, city, zip code, and phone number of each item in the dropdown list. I need to set this up so that when a selection from the dropdown list is made, the each individual information about that selection (address, city, zip code, phone, etc.) is automatically filled in its own textfield. How can I do this? Thank you in advance for providing detailed answer.
