I'm looking for a javascript that would do the following:
 
0) If file name does not equal "NomPaperMC", close.
1) On Open: Ask User to agree or not agree to allow document to find system resources.
2a) If Yes - Then Document Finds MAC address (or something similar that is unique to a computer) and saves it.
2b) If No - Dialogue to user, you can order pre printed forms from our website at blah blah blah.com
3) User then inputs data into Form and Saves document.
4) On the next time the user opens the document, if the MAC address matches then allow user to use PDF form.
4b) If MAC address doesn't then dialogue: This document must be opened in same computer that it was originally opened in. Contact Rigo at XXX if you need further assistance.
 
Please let me know if you think this is possible, and the cost of the script.

I don't think what you describe is possible using JavaScript, and anyway you should post this on the Job Board.
Also, you should provide a proper email so people can contact you, not something cryptic as "Rigo at XXX".
