Is there a sample script one can use (modify) to either create a table of contents page (from a group of pages within a PDF), or fill in designated fields on a page, the page number & page title from each of the pages within the PDF?

Yes, you can write a script to do these things. In fact, these are fairly standard automation tasks, but it is not a simple script. Read the Acrobat JavaScript Reference, get some training, or hier a consutant or developer.
