I have several fairly simple forms that need to be developed/created for advanced practice providers in the hospital, unrelated to medical records--administrative functions and information gathering.
There would be 3-4 forms total, each only a page or two.
Need to be well designed (I have a rough idea of layout) and have be:
1. pdf documents
2. have fillable forms
3. repeat some fillable fields (e.g. - name) in other areas of form so user does not have to re-type same information.
