cross-reference problem after conversion from Word to pdf

I am currently using Microsoft Office Word 2003 SP2 and Adobe Acrobat 7.0 Professional. My problem involves cross-references that exist in the Word document but disappear after conversion to pdf file. My usual step of adding cross-references are the following: Insert  Reference  Cross-reference  Reference type: Footnote  Insert reference to: footnote number (formatted). After conversion to pdf, some of the cross-references will disappear and I will have to re-cross-reference in the Word document before converting to pdf. My questions are as follows:

1. Do you have any ideas on what is causing this problem?
2. Do you have any suggestions for possible solutions?
3. Would a third program like Endnote help in this situation?

We noticed that Acrobat X does not convert all cross references to figures and tables within the same Word document to links. We cannot see any specific pattern as to which ones are 'left out'. We use Acrobat X and Office 2007 under Windows 7. I installed the latest update, which did not address this issue. Did anybody encounter the same? If yes, is there any workaround?
