Making an existing PDF form editable

Hi! I have an existing PDF form that was created by one of my insurance companies. I would like to be able to type into it. Can anyone help me with this?

I've already downloaded Adobe Acrobat X Pro - the trial version. I've been trying to figure it out but NO.

HELP!

Though the trial version will keep irritating you by notification popups but still you can Save a copy of that form and then try to make changes.
