Using Word 2003 and Acrobat Standard 8, the option to "Update fields" in the Word doc is disabled for the TOC, Index, and all the cross-references after I've converted the doc to PDF. Normally, F9 and a context-menu option "Update Fields" is available in Word, but the menu option is grayed out and F9 doesn't work after PDF conversion.
If a new TOC or cross-reference is added to the doc, the update function still works. That is, they work until the doc is converted.
Not sure if it's related, but the cross-references also show something surprising: an Adobe font called ZWAdbobeF appears for the font style name when the page number (cross-ref) is highlighted.

The PDF conversion appeared to properly complete its processing.
I tried to update the fields both ways, individually and en mass (Ctrl+A), neither worked. I had to remove and recreate all the fields: TOC, Index, and cross-references to make sure they were updated.
It is possible the doc was originally created with Word 2000. Its template was definitely created in Word 2000.
And, on top of this problem the PDF exhibited another: extra characters appeared in the Headers, which are also based on fields (STYLEREF) in the Word doc. We have seen this bug with Acrobat 8 before, but we fixed it by making sure the Heading styles in Word were named as expected. In this case, the styles are correctly named, so I'm stuck with another issue I don't know how to resolve.
Should I uninstall Acrobat 8 and reinstall Acrobat 6?

There have been other reports of issues with hyperlinks from Word files not converting properly but not issues with actual commands not performing properly in Word. Do you have other Word macros installed that might be conflicting with the PDFMaker macro? It seems that there were significant changes to hyperlinks in Word between versions 2000 and 2003. The syntax of these links doesn't convert properly to PDF (you can also see the issue if you save the Word file as RTF and view the syntax). However, the problem seems to be corrected if you re-create the hyperlinks as you have done.
