Can the template of the Preflight Summary Report be customized to include company logos and/or contact information like the regular or minimum preflight report from Pitstop?

Might be able to. Try here: C:\Program Files\Adobe\Acrobat 10.0\Acrobat\plug_ins\Preflight\

Those folders/files do not appear to control the placement of the "Preflight Summary Report for: *****.pdf" type, which I would like to have moved down 3/4" so that a logo and other type can be placed above it.

I do not believe that you can do that.
