There could be a couple of reasons for this. First, are you reader-enabling the form before sending it out so recipients using the free Reader are able to save the data? This happens automatically if you use the Distribute command. Second, make sure your recipients aren’t using Mac Preview to fill out the PDF form, this can create blank fields as well.

6, 2014-08-28 28, 2014I created a form using form wizard. once the data is entered and the form is emailed it comes through blank.
the original saved copy always remain in tact, its only when its emailed that it happens
