In this tutorial, learn how to create fillable PDF, including creating check box form fields in fillable PDF forms or online forms. This summary is excerpted from chapter 11 of Creating PDF Forms for Government by Ted Padova. Follow the link below to download the complete 15-page sample chapter.

This sample chapter from the free Creating PDF Forms for Government guide explains the various types of check box fields you can create while designing a web or online form in Adobe Acrobat XI.

This PDF sample chapter also includes a video of creating check box form fields.

In Acrobat XI you can design a group of form check boxes that permit only a single response from among the group, and they are considered mutually exclusive. When one check box is checked, all other check boxes within a group are cleared of data and remain unmarked. You can also design a series of form check boxes where more than one response is needed within a common group of fields.

Chapter 11 of The Creating PDF Forms for Government guide explores different check box form field techniques including:

  • Creating Mutually Exclusive Check Boxes
  • Copying/Pasting Check Box Fields
  • Creating Non-exclusive Check Box Fields
  • Working with Mutually Exclusive Fields in Tables

Hi Patty, thanks for following up. I discovered that the link was taking me to the iOS version rather than the PC version. By going onto the site I was able to get the PC version.
